AQTIA Inc. (hereinafter referred to as ‘we’, ‘our’, ‘us’) is a firm providing information system development (incubation) for new business development and we are strongly aware that keeping personal information confidential is our important duty. We, as a whole company, hereby declare that we have built the following policies for 00personal information management system and we will continue the improvement while always following the latest IT technical trends, changes in social requirement and changes in management environment.
1. We will acquire, use and provide personal information in a proper way and never use it for any other purposes except for the specified scope. Plus, we will take measures for that.
2. We will comply with laws, regulations set by Japan and other criteria regarding handling personal information.
3. In order to prevent the risk of the leak, loss or any damages of the private information, we shall always improve our security system continuingly by setting up proper resources to form up a rational safety measures and prevention. Also, in case of emergency, the company will take corrective action promptly.
4. We will respond with a prompt and faithful manner to any complaints and consultation concerning handling of personal information.
5. The personal information protection management system will be constantly reviewed and improved in accordance with the changes in circumstances of the company.
This policy will be distributed to all employees for dissemination while posting this policy on our website, pamphlets, etc. and take measures that anyone can obtain at any time.

Public Announcement of the Purpose of Use for Personal Information
We announce that we will use personal information within the scope of the following purposes, based on Article 18(1) of the Act on the Protection of Personal Information (Personal Information Protection Act).

4. About our authorized personal information protection organization
We do not have an authorized personal information protection organization.
Procedures for disclosure of retained personal data
For the retained personal data, you can request for using purpose announcement, disclose, content correction, add or delete, suspension of use, deletion and suspension of provision to third parties” (disclosure, etc.). We will respond to requests for disclosure of personal information by the following procedure.
1. Reception for Disclosure Requests
(1) Please contact the below address. We will send you a ‘designated form’ to you. When you received, please agree to our policy of ‘handling personal information’ and bring it or send it to the below address by post.

(2) To verify your identity, please bring or send one of the following documents together with the ‘designated form’ in (1).
– A copy of an official identification such as driver license, passport, etc. (the ones with your name and address written)
– A copy of the residence certificate (released within 30 days prior to the date of the request for disclosure)
(3) In case a representative runs the procedures, please bring or send one of the following documents together with (1) ‘designated form’, (2) ‘official identification’ by post.
(A) Identification of the representative
- – A copy of an official identification such as driver license, passport, etc. of the representative (the ones with your name and address written)
- – A copy of the residence certificate of the representative (released within 30 days prior to the date of the request for disclosure)
- – In case an attorney is a representative, please bring the document with registration number written.
(B) A document proving the right of representation (such as authorization paper)
- – If the domicile of residence is shown in the document certifying the person or the representative, you can delete it.
- – We take a responsibility to dispose the document certifying the person or the representative within 2 months after receiving.
2. ‘Disclosure Request’ fee and collection method
For the request of announcement or disclosure of purpose of use, the following amount (including fee for our reply) will be charged for each request. Please enclose a postal money order of following fee when sending request document.

3. Consideration Result Notification
We will notify the result as soon as possible by the following methods.
– In case of the use purpose announcement of personal information, we will send the paper forms by post or hand them directly.
– In the case of correction, notification or deletion of personal information, suspension of use, deletion of personal information and suspension of provision to third parties, we will contact through telephone or address by one of the following methods.
– Send the paper forms to the registered address by post.
– Send the paper forms to the registered fax number by fax.
– Send the answers to the registered e-mail address by e-mail.
– Directly call the request person, and verbally answer.
